Basic Life Insurance

Stork Materials Technology provides you with Basic Term Life Insurance through Unicare, equal to 2 times your annual base salary to a maximum benefit of $300,000. The plan also provides living benefits in the event of terminal illness, up to 50% of the death benefit up to $125,000.

Supplemental Life Insurance

You may elect to purchase Supplemental Term Life insurance at group rates provided by Unicare. You pay for this coverage with after-tax dollars through convenient payroll deductions. You may purchase benefits in the amount of 1, 2, or 3 times your basic annual earnings, up to a maximum of $250,000. This coverage is portable for two years and a conversion option is also available.

You may also purchase $10,000 for your spouse and $5,000 (6 months to age 19/25) or $1,000 (Birth to 6 months) for your children.

You may apply for Supplemental Life insurance at any time during the year. Coverage is subject to medical underwriting approval by Unicare.
